Out-of-Network: We can provide you with a Superbill for your insurance to reimburse you. It would be helpful for you to contact your insurance carrier to learn more about your specific coverage (i.e. reimbursement rates for out-of-network providers, deductible amount, if pre-authorization or referral is required, how many sessions are covered, required forms for reimbursement).
cancellations
Please be aware that if you do not provide at least 24 hours notice of a cancellation or you do not show for a scheduled appointment you may be charged $50, unless we both agree that you were unable to attend due to circumstances outside your control.
